The South West Paediatric Diabetes Network is becoming very active and is very keen to gather the views of parents and patients (12 years and older) on what the issues are with having Diabetes but also what they think of the service they receive.

Below are the two SurveyMonkey links for patient and parent surveys in the South West. 

If you attend any of the hospital below,please follow the links and fill in the on-line survey. If you have already completed the survey thank you.
